There are many factors involved but no one knows the formula used to calculate results, that would be cheating :D.
I believe that formation is the most important. (after ratings)
If you play 3-5-2 and your opponent plays 4-3-3 then you should dominate the centre of the park and thus possession and should , in theory, stop him from scoring. However, if he gets through your brick wall of midfielders then there is a 3vs3 situation in your defense which is not advisable. So, in this situation you could see yourself have ~65% of the possession and yet still lose 3-0 due to a poor defense. This emans that to have 3 in the back you must have a good defensive midfielder making defensive runs and a solid back three in order to hold off opposition strikers.
Then if you are attacking then you have a 2vs4 situation in which two defenders can go zonal and the other two can go man to man and this should defeat your attack, in theory. But, if you have good wingers that can support your strikers and draw away defenders then you will have a chance of scoring.
So I hope you can see that formation is just as important as player ratings. I have seen Div4/5 teams defeat Barcelona/Madrid purely due to having a formation that can counter the opponents.
